What to Expect at Your Buttock Augmentation Surgery
Getting gluteal implants is a major surgery requiring preparation and an extensive recovery period. Here’s an idea of what you can expect:
Consultation
The first step is the consultation with a board-certified plastic surgeon. During this meeting, you will discuss your surgical goals, medical history, and lifestyle. Dr. McFadden or Dr. Felice will evaluate your general health status and take detailed body measurements. They’ll assess the size of your pelvis, your height, and your desired degree of augmentation, and discuss the best course of treatment for you. It’s crucial to be completely candid during this consultation to ensure the success and safety of the procedure.
Preparation
To ensure the safety and success of the procedure, you’ll need to take a few steps in the weeks leading up to surgery.
Stop smoking at least six weeks before the surgery. Smoking causes blood vessels to constrict, which can interfere with the healing process and increase the risk of complications. This includes all forms of nicotine, such as e-cigarettes and nicotine gum.
Avoid taking aspirin, certain anti-inflammatory drugs, and any medications that affect blood clotting as they can increase bleeding. It’s recommended to stop these medications and supplements at least two weeks before the surgery.
You may be asked to get a blood test as part of the preoperative evaluation. If you come down with a cold, fever, rash, or any new medical problem close to your surgery date, you should notify your surgeon immediately.
Day of surgery
Do not eat or drink after midnight the day before your butt augmentation surgery. If your procedure is performed on an outpatient basis, arrange for someone to drive you to and from surgery and to stay with you for at least the first night following the procedure.
Surgical procedure
The day of your surgery, we’ll review your goals and the surgical plan. Once you’re comfortable, you’ll receive anesthesia; depending on your health, the specifics of the procedure, and your comfort and preferences, this may be general anesthesia, intravenous sedation (twilight anesthesia) or an epidural.
Your surgeon will then make incisions at predetermined locations chosen according to your anatomy and preferences. These incisions may be placed at the top of each upper buttock or the bottoms of the gluteal creases, or a single incision may be made placed down the center of the sacrum.
Next, a silicone implant specially made for the buttocks is inserted either in the muscle or above the gluteal muscle on each side. The method for inserting and positioning implants depends on the type of implant, degree of enlargement desired, your body type, and your surgeon’s recommendations.
The incisions are then closed with layered sutures in the buttocks tissue and with sutures, skin adhesive, or surgical tape to close the skin.
Recovery Timeline
Downtime after buttock augmentation varies among individuals, but here’s a general timeline:
2–3 Days Following Surgery: Moderate to severe pain and tenderness in the area is common and will be managed through pain medication. You should refrain from sitting down, bending over, or sleeping on their back until cleared to do so by your surgeon.
3–4 Weeks Following Surgery: Most bruising or swelling in the area should be resolved. The implants, however, may continue to feel firm and tight. You may be cleared to perform light to moderate exercise.
5–6 Weeks Following Surgery: Bruising and swelling to be mostly resolved, revealing the final results of the procedure. Surgeons usually clear patients to resume all forms of activity including strenuous exercise.
Throughout the recovery process, it’s crucial to follow all post-surgical instructions provided by your surgeon to ensure a healthy recovery and optimal results. Follow-up appointments with Dr. Thomas McFadden will ensure that you’re healing properly and that the implants are settling as expected.

Ideal Candidates for Buttock Augmentation
Men and women between the ages of 18 and 65, at or near their ideal weight, and in good health can be good candidates for buttock augmentation. The procedure can address:
- Flat or poorly defined buttocks
- Lack of volume due to aging or weight loss
- Asymmetry between the two buttocks
Contraindications for this treatment include medical conditions such as obesity, diabetes, and high blood pressure. Patients with high levels of skin ptosis (sagging) may require SKIN EXCISION SURGERY in addition to implants in order to achieve the desired effect. If you are pregnant, you should wait before getting implants or other forms of cosmetic surgery.
Frequently Asked Questions
What is the difference between fat transfer and implants in buttock augmentation?
Fat transfer, commonly known as a Brazilian Butt Lift or BBL, involves grafting your own fat from other areas of your body to augment the buttocks. Implants, on the other hand, are silicone-based and are surgically inserted. At Advanced Cosmetic Surgery, we find that most people are better candidates for gluteal implantation.
How long do butt implants last?
Implants may last many years but will eventually require replacement or adjustment. Patients can usually expect their implants to last around 10 years.
Is buttock augmentation painful?
Most patients experience some discomfort after the procedure, which can be managed with prescribed pain medication. During the procedure, anesthesia is used to ensure you’re comfortable and pain-free.
Can I combine buttock augmentation with other cosmetic procedures?
Yes, it’s common to combine buttock augmentation with other procedures such as LIPOSUCTION, a TUMMY TUCK, or BREAST AUGMENTATION. Combining procedures can be a cost-effective way to achieve your desired look in fewer surgical sessions.
Are there any risks associated with buttock augmentation?
As with any form of plastic surgery, there are some risks involved. A review of more than 500 procedures found the most COMMON COMPLICATIONS after surgery were wound opening, fluid accumulation, and implant palpability, i.e. the implant could be felt through the skin.
Choosing an experienced, board-certified plastic surgeon like Dr. Thomas McFadden or Dr. Peter Felice can significantly minimize the risks of adverse effects. Should complications arise, an qualified surgeon will help ensure your smooth recovery.
